Improving the Access and Utilization of Palliative and Hospice Care in the Intensive Care Unit
Patients in the intensive care unit, need more timely access to palliative and hospice care. The aim of this project was to increase the appropriate and early timeliness of palliative and hospice care consultations in the intensive care unit. Education was provided for all healthcare providers on the unit and the use of a trigger-based diagnosis system was utilized to identify patients with the potential for unmet palliative and hospice care consultations.
